To Request an Evaluation

  1. Email [secure email] or call [phone] with case number, court, custody status, and next hearing date. Availability confirmed within [one business day].

  2. Send the court order (or retention letter if counsel-retained).

  3. Send the referral sheet [download link] and records. Minimum to schedule: criminal complaint and arrest report. Helpful: jail medical and mental health records, prior psychiatric records, prior competency reports, collateral contacts.

  4. Interview scheduled within [X business days] of complete referral.

  5. Report delivered to the court, defense counsel, and the prosecutor by secure transmission within [X business days] of interview.

Report Format

Opinion on page one. Then: forensic advisement, itemized sources, relevant history, medications and course of treatment in custody, assessment of each statutory prong with the defendant quoted directly, MSE, DSM-5-TR diagnostic impression. Same structure every report. Opinion is independent of referral source.
